The Sams Clinic accepts referrals at any time without a referral form. We also will refer your client back to you, and we routinely do radiograph reviews at no charge. We do ask that whenever possible you email or fax pertinent lab work to us prior to your patient's appointment or alternatively have your client bring it to their pet's initial appointment with the clinic or on the day of surgery. Please have your client bring us any digital radiographs at that time as well or you may send them to us. Please do not email digital x-rays; the resolution of the image will not be high enough. Advanced CT Imaging at The Sams Clinic

If you determine that a patient in your care needs a CT scan and you refer your client to The Sams Clinic, we'll refer your client back to you for all discussion of their pet's care and treatment. Our CT Scanner is the Luminys PQ2000, a fourth generation ultrahigh resolution helical computed tomography scanner.

    How to Send a Patient for a CT Scan

    We don't require a referral, but do ask you to comply with the following so that we can provide your patient with the best and the best clinical interpretation of his or her scan:

     

  • Fill out a CT Referral Form, downloadable here or available by fax from our receptionist, and return the completed form to us. It's important that you tell us which part of the patient's body is to be scanned and whether or not to use contrast.
  • Send all pertinent x-rays with the patient on the day of the scan. They will help us take the best scan possible. We also send these x-rays along with the CT scan to the radiologist.
  • Provide a complete patient history. This is required by the radiologist and to help interpret the CT scan. We overnight the results to your Radiologist using Fedex.
  • Provide blood work results (CBC, Chemical) dated within two weeks prior to scan.
  • Share your anesthesia evaluation via the referral form and discuss any concerns you may have with Dr. Hawley, our anesthesia specialist.
